Coagulation factors can be preserved in plasma frozen soon after collection and stored at or below -25°c. In that state fresh frozen plasma (FFP) and cryoprecipitate can be kept frozen for up to 3 years. The products are usually used to prevent or stop bleeding due to deficiencies of coagulation factors which may be brought about in various ways.
Indications for transfusion are as follows:
FFP:
• Major Haemorrhage
• Bleeding INR > 1.5, APTR >1.5
• Pre op INR >1.5, APTR >1.5
• Liver Impairment and pre op INR >2.0
• TEG/ROTEM Indicated
Cryoprecipitate:
• Major Haemorrhage
• Bleeding Fibrinogen <1.5g/l
• Pre op Fibrinogen <1.0g/l
• TEG/ROTEM Indicated
Prior to the issuing of Blood and Blood components patients are required to have had their blood group tested / confirmed on samples taken at two separate venepunctures. If this has not been carried out then group AB or A HT negative components can be thawed in emergency situations.
